Participation in any fossil collecting activity and use of any tools involves a risk of accidental injury despite all safety precautions. Having been informed of the proper safety precautions such as safety glasses/goggles, gloves, and hard hats, I/we, as an individual or as a parent or guardian of the participants named herein, assume all risks and responsibilities incidental to any and all activities, and release from responsibility and agree to indemnify and hold harmless CoolClay’s Fossil Expedition, Clayton Kern, the owners of the fossil sites, and anyone else other then myself for any injury to me or my children or family members occurring during his/her/our participation in any activity or use of any of the tools provided by CoolClay’s Fossil Expedition/Clayton Kern. If the signee below is under 18 years old, a parents signature is also required. The parents signature also gives CoolClay's Fossil Expedition/Clayton Kern expressed consent for emergency medical care to be administered in case of an emergency.

By signing below I understand and hold myself fully responsible and accountable for any incidental accidents whatsoever that may happen, and if I decide not to follow the proper safety precautions, and wear the proper safety equipment then it is my responsibility and no one else’s.
